Trying to Model Radiant Cooling, Getting Degenerate surfaces

We are trying to create a Radiant Cooling model. First we tried with our whole building model and ran into lot of errors, so we thought of trying it for only 3 zones first. We created the model and modeled the radiant slab using Open Studio building component Library. From there we simulated it using Energy Plus. However, every time we are getting Degenerate Surfaces and simulation stops due to fatal errors. We are not able to ascertain what those surfaces are and why they are getting created. Based on another thread, we tried deleting those surfaces. But it did not work and Simulation collapses due to fatal errors. It's a very simple model and we have tried removing orphan objects, etc. Anyone has any suggestions. This is our first Radiant Slab Model.

Degenerate surfaces usually do not terminate your simulation, other fatal errors are what is doing it.
